Photography Consent
I grant permission for BAM Mountain State to photograph my child during activities, and I understand these photos may be used, without restriction, for promotional materials, social media, or related purposes.

Liability Waiver
I, the parent/guardian of the minor named herewith, consent to participation in BAM Mountain State programs. I release and hold harmless BAM Mountain State, its successors, employees, agents, and officers from any and all claims, demands, or damages arising from participation, including personal injuries, property damage, or medical treatment. I agree to indemnify BAM Mountain State and its representatives for any costs, including attorney’s fees, resulting from the minor’s intentional or grossly negligent actions during participation. I confirm that she/he is in proper physical condition and will provide required equipment for participation. In case of emergency requiring medical attention beyond first aid, I authorize a physician or hospital designated by BAM Mountain State to provide care.
